Die Implantat-Abutment-Verbindung steht als Einflussfaktor für das marginale periimplantäre Knochenniveau im Fokus der klinischen Forschung. In dem Beitrag werden alle klinisch relevanten Informationen zu den verschiedenen Implantat-Abutment-Verbindungen zusammengestellt. Es gibt Hinweise darauf, dass Implantat-Abutment-Verbindungen mit eingebautem Platformswitch einen positiven Effekt auf das periimplantäre Knochenlager haben und damit eine geringer ausgeprägte Knochenremodellierung auslösen. Somit ist vor allem im ästhetisch hochsensiblen Versorgungsgebiet (Frontzahn- und Prämolarenregion) diese Art der Verbindung indiziert. Die bakterielle Kontamination von Implantatinnenräumen im Rahmen implantatprothetischer Versorgungen ist allerdings schwer vermeidbar. Purpose: To test a newly introduced implant-abutment material combination against bacterial endotoxin leakage in a human whole blood assay. Materials and Methods: Two dental implant systems with internal connections and the following material combinations at the implant-abutment interface (IAI) were used (implant material/abutment material): yttrium-stabilized tetragonal zirconium dioxide (Y-TZP)/ polyetherketoneketone (PEKK), and titanium (Ti/Ti). Test implants were inoculated with lipopolysaccharide (LPS) and sealed and submerged in human whole blood. Untreated implants served as the control groups. Changes in gene expression levels of inflammatory markers indicating LPS leakage were assessed after 1, 8, and 24 hours using quantitative real-time polymerase chain reaction. Results: In the Y-TZP/PEKK test group, a significant influence of the implant system (P < .001) on increases in gene expression indicating leakage were detected after 8 hours for TLR-4 and after 24 hours for interleukin 1-β and nuclear factor kappa-lightchain- enhancer of activated B-cells (NF-κB), indicating microleakage of LPS at the IAI. In the Ti/Ti test group, differences in gene expression were found only for NF-κB after 8 hours. Conclusion: The internal hexalobe IAI of two-piece dental implants fabricated from Y-TZP and PEKK do not prevent LPS molecular microleakage.

Die Verwendung von dentalen Titanimplantaten gilt aufgrund einer vorhersagbaren Osseointegration als sicheres Standardverfahren in der Zahnmedizin. Jedoch sind viele immunologische Prozesse, auf denen die Osseointegration beruht, noch nicht vollständig erforscht. Vor allem der Einfluss der Implantatoberflächen auf die immunologische Entzündungsreaktion in der Frühphase der Osseointegration steht derzeit im Fokus der Forschung. Die Konditionierung von Titanoberflächen dentaler Implantate mit physikalischem Plasma ist ein vielversprechendes Verfahren, um die Implantatoberfläche zu modifizieren und somit einen Einfluss auf die Osseointegrationsprozesse zu nehmen. Forschungsergebnisse der letzten Jahre scheinen klinisch relevante Effekte hinsichtlich einer modifizierten immunologischen Wirtsreaktion, positiver zellulärer Effekte und klinisch messbarer verbesserter Osseointegration zu demonstrieren. Weiterhin spielt die Anwendung von kaltem atmosphärischem Plasma für Dekontaminationsprozesse im Rahmen der Periimplantitistherapie eine zunehmende Rolle in der Wissenschaft. Für die dentale Anwendung kommen derzeit kaltes atmosphärisches Plasma (CAP: Cold atmospheric plasma) und Niederdruckplasma (LPP: Low pressure plasma) infrage. Die chirurgisch-prothetische Sofortversorgung des unbezahnten Kiefers mit vier Implantaten ist eine interessante Behandlungsvariante, die selbst im atrophierten Kiefer die Möglichkeit der festsitzenden Versorgung unter häufigem Verzicht augmentativer Maßnahmen bietet. Dennoch ist dieses Verfahren nach wie vor im deutschsprachigen Raum umstritten. Die mechanische Fixierung der Implantate durch die Ausnutzung anatomischer Gegebenheiten sowie die Verwendung einer angulierten Implantatposition für die Schaffung eines stabilen Abstützungspolygons stellen hierbei die Grundlage des Verfahrens dar. Neben einer sorgfältigen Diagnostik und Planung spielen ethische Fragen zum Erhalt oder der Extraktion der vorhandenen Restbezahnung eine große Rolle für die Therapieentscheidung. Die verfügbaren klinischen Daten zeigen erfolgreiche Überlebensraten der Implantate und des Zahnersatzes bei gleichzeitig erhöhten technischen Komplikationsraten in kurz- bis mittelfristigen Beobachtungszeiträumen. Der Langzeiterfolg dieses Verfahrens beruht neben der exakten chirurgischen, prothetischen sowie der labortechnischen Umsetzung auch auf einer regelmäßigen und strukturierten Nachsorge, um biologische und technische Misserfolge zu vermeiden. Im Rahmen dieses Beitrags werden die Grundprinzipien, der klinische und labortechnische Behandlungsablauf sowie die verfügbare Evidenz des Verfahrens zur chirurgisch-prothetischen Sofortversorgung des unbezahnten Kiefers mit vier Implantaten beschrieben. Aufgrund der Prominenz der Frontzähne im Gesichtsschädel haben Frontzahntraumata eine relativ hohe Prävalenz. Sie stellen den Zahnarzt vor die schwierige Aufgabe, die betroffenen Zähne in erhaltungswürdig oder nicht erhaltungswürdig zu kategorisieren. Wenn Zähne sich als nicht erhaltungswürdig herausstellen, müssen sie in einem ästhetisch anspruchsvollen Bereich ersetzt werden. In diesem Artikel werden, abhängig von verschiedenen Ausgangssituationen, endodontische, prothetische und chirurgische Versorgungskonzepte vor gestellt. Die Risiken und Möglichkeiten der einzelnen Therapieoptionen werden anhand von Fallbeispielen diskutiert. Im Rahmen einer implantatprothetischen Versorgung des teilbezahnten Oberkiefers besteht im Seitenzahnbereich aufgrund der anatomischen Nachbarschaft zu den Sinus maxillares häufig ein vertikales Defizit an ortsständigem Knochen. Bedingt die prothetische Planung eine Implantatpositionierung im defizitären Bereich des Alveolarfortsatzes, muss eine der Situation angemessene augmentative Maßnahme gewählt werden. Neben dem externen Sinuslift (laterales Vorgehen), der in Fällen eines ausgeprägten vertikalen Knochendefizits eine sichere Form der Sinusbodenelevation darstellt, deckt der interne Sinuslift (transalveoläres Vorgehen) ebenfalls einen breiten therapeutischen Bereich ab. Hierbei hat der Patient einen deutlichen Vorteil aufgrund der geringeren Invasivität des Eingriffs zu erwarten. Weiterhin lässt die positive klinische Bewährung kurzer Implantatbauformen im distalen Seitenzahnbereich der resorbierten Maxilla die unbedingte Notwendigkeit zur Augmentation erst später entstehen, als dies vor einigen Jahren noch der Fall war. Purpose: Implant-supported dental prostheses are based on the principle of osseointegration, and the success of dental implantation depends on adequate formation of this intimate bone-to-implant contact. The application of heat during the drilling procedure leads to a rise in intraosseous temperature at the prospective implant site, which may result in various complications. The purpose of the present study was to compare the ability of thermocouples and infrared thermography to detect changes in intraosseous temperature during dental implant site preparation. Materials and Methods: Standardized bovine costae bone blocks were used to simulate the cortical bone of the human mandible. Steel implant form drills with a diameter of 3.0 mm were used. Two types of irrigation systems were used (external cooling only and combined internal and external cooling). Drilling was performed at a constant speed (1,200 rpm). Changes in intraosseous temperature were evaluated using Type T Cu-CuNi thermocouples and an infrared thermography camera system at contact pressures of 5 and 20 N. Results: Infrared thermography detected significantly greater increases in intraosseous temperature (ΔT) than thermocouples for all tested combinations of cooling system and contact pressure (P ≤ .0001). Conclusion: These results suggest that thermography more accurately reflects intraosseous temperature changes during implant site preparation than thermocouples. Purpose: To test the adhesion properties of live gingival fibroblasts to three different implant abutment materials after five different cleaning procedures. Materials and Methods: Highly polished discs of lithium disilicate (LS), zirconium dioxide (Zr), and titanium alloy (Ti) were fabricated. The specimens were cleaned by one of five different methods: steam (S), argon plasma (AP), ultrasound and disinfection (UD), ultrasound and sterilization in an autoclave (UA), or photofunctionalization with high-intensity ultraviolet light (PF). Cell detachment force (adhesion) was measured by single-cell force spectroscopy, which is a method to quantify cell adhesion at the single cell level. Data were statistically analyzed using parametric tests (analysis of variance [ANOVA], t tests). Results: Cell detachment forces in the low nN regime were recorded in all experiments. Significant differences in cell adhesion on the different materials were found as a function of the cleaning method (P ≤ .0001). For LS abutments, no significant differences between the cleaning methods could be found (P > .05). For Zr specimens, the AP method showed the highest cell detachment forces, followed by UD, PF, S, and UA (S/UD, S/UA, S/PF, AP/UD, and UD/PF were not significantly different from each other). For Ti abutments, UD showed the highest cell detachment forces, followed by S, AP, and UA/PF (S/UD, S/UA, S/PF, AP/U, and UA/PF were not significantly different from each other). Conclusion: All cleaning methods provided comparable cell detachment forces for LS abutments. AP/PF or ultrasonic cleaning were the most suitable methods for strong cell adhesion on Zr. UD provided the best cell adhesion for Ti. Zahnarztbesuche lösen trotz weitgehend schmerzfreier Behandlungen bei 60-80 Prozent der Patienten Angstgefühle aus. Bis zu 20 Prozent leiden unter einer pathologischen Zahnbehandlungsangst, die bei 5 Prozent mit einem Vermeidungsverhalten vergesellschaftet ist1. Aufgrund der großen Prävalenz stehen viele Zahnärzte früher oder später vor der Herausforderung, dieser anspruchsvollen Patientengruppe eine Behandlung zukommen zu lassen. Der Umgang erfordert Feingefühl, eine gut durchdachte Praxisorganisation und das Angebot spezieller Narkoseverfahren, um Behandlungen trotz diverser Hemmnisse zu beginnen und erfolgreich zu beenden.

Purpose: The main goal of this in-vitro study was to evaluate whether tensile strength and retrievability of cemented implant-retained crowns correlate when using artificial aging. Materials and Methods: A total of 128 crowns were fabricated from a cobalt-chromium alloy for 128 tapered titanium abutments (6 degrees taper, 4.3 mm diameter, 4 mm length, Camlog). The crowns were cemented with glass-ionomer (Ketac Cem, 3M) or resin cements (Multilink Implant, Telio CS Cem [Ivoclar Vivadent], Retrieve [Parkell]). Multilink Implant was used without priming. The experimental groups were subjected to either 37,500 thermal cycles between 5°C and 55°C, 1,200,000 chewing cycles, or a combination of both. Control groups were stored for 10 days in deionized water. The crowns were removed with a universal testing machine or a clinically used removal device (Coronaflex, KaVo). Data were statistically analyzed using nonparametrical tests. Results: Retention values were recorded between 31 N and 362 N. Telio CS Cem showed the lowest retention values, followed by Retrieve, Ketac Cem, and Multilink Implant (P ≤ .0001). The number of removal attempts with the Coronaflex were not significantly different between the cements (P > .05). Thermal cycling and chewing simulation significantly influenced the retrieval of Retrieve Telio CS Cem, and Ketac Cem specimens (P ≤ .05). Only for Multilink Implant and Telio CS Cem correlations between removal with the universal testing machine and the Coronaflex could be revealed (P ≤ .0001). Conclusions: Ketac Cem and Multilink Implant (without silane) can be used for a semipermanent cementation. Retrieve and Telio CS Cem are recommendable for a temporary cementation.
