As stability is a key determinant of successful regeneration, this paper introduces tack engagement as a novel alternative to conventional suturing techniques, aimed at improving the positioning and fixation of autogenous tissue grafts. Rigid tack engagement, a widely utilized technique in guided bone regeneration (GBR) protocols for securing membranes, offers a stable and time-efficient solution that mitigates the risks of early graft displacement resulting from suturing or delayed volume changes due to physiological healing processes or unintentional patient oral habits. Subperiosteal tissue tacking represents a promising alternative technique for managing small to moderate tissue defects, mitigating various biological and practical obstacles. Within the limitations of this current report, tacks provide a variety of practical benefits for tissue fixation, rendering them particularly useful in staged implant placement procedures. The aim of this report is to describe a new sling suturing method with papillary anchorage that is clinically applicable within the available conventional root-coverage tunneling procedures. Although caution is advised to not increase excessive tension on the fragile papilla tips, as they provide coronal and palatal suspension, the tension used with this method ascertains a firm and stable connection for the entire buccogingival graft complex into the horizontal and vertical dimensions of both single and multiple recession defects. This technical note describes the “trapezoidal sling suture” technique, which allows fixation and stabilization for the graft and flap around both natural teeth and implants. Purpose: To evaluate the clinical survival and impact on oral health–related quality of life (OHRQoL) of immediate loading of implant-supported full-arch CAD/CAM–milled PMMA interim restorations. Materials and Methods: This was a practice-based prospective cohort study performed in a Portuguese dental clinic. Eleven patients received a total of 17 CAD/CAM fully implant-supported, screw-retained, full-arch restorations with milled PMMA and were followed up for 1 year. The primary outcome was prosthesis survival, determined with a modification of the California Dental Association score and a Kaplan-Meier survival function analysis. The secondary outcome was the patient quality of life, as defined by application of the OHIP-14-PT questionnaire and standardized effect size variation between two visits. Significance was set at 5%. Results: Survival probability at 12 months was 76%, complete fracture of the prosthesis occurred in 17.6% of the cases, and small fractures without lab involvement occurred in 5.9% of cases. There was a significant improvement in OHRQoL between visit 1 and visit 4. The mean difference and effect size for total OHIP-14-PT score were –32.91 ± 3.68 and 3.66 (95% CI –1.83 to –5.80) respectively (P < .001* Wilcoxon matched paired rank test). Conclusions: Fullarch implant-supported CAD/CAM—milled PMMA interim prosthesis seem to be a viable approach with good survival rate and great impact on patient OHRQoL.

Aim: The aim of the present preliminary study was to observe and make a histologic comparison of connective tissue grafts (CTGs) harvested from the lateral palatal mucosa through the use of two different harvesting techniques. Materials and methods: Three patients were enrolled in the study, providing six standardized CTGs. One well-experienced periodontist collected the replacement grafts using two different methods. After outlining the grafts to a fixed dimension, the graft on one side was deepithelialized by a round coarse bur intraorally before harvesting. The graft on the contralateral side was obtained by harvesting from the palate first; subsequently, deepithelialization was performed extraorally with the aid of a no. 15c blade. After finalization, histologic evaluation was performed. Results: No apparent differences were found between the two observed techniques in terms of graft thickness, proportion, and composition. After deepithelialization, epithelial remnants were clearly evident in five out of six cases. Despite being more technique sensitive, the removal of epithelium by bur scored better. Proper graft handling and graft regularity are described as advantages of the more conventional epithelial excision by blade. Conclusions: Despite the wide use and broad variety of commonly applied techniques of graft deepithelialization, the present authors assume that full excisions with the use of a blade are hardly ever achieved. Despite the unpredictable retrieval of epithelium by blade, graft handling and graft regularity can be proposed as the biggest advantages. On the other hand, the presented novel in situ deepithelialization with a round bur seems to be more predictable.

Ziel: Ziel dieser vorläufigen Studie waren die Bewertung und der histologische Vergleich von Bindegewebetransplantaten (BGT), die unter Verwendung von zwei verschiedenen Techniken aus der lateralen Gaumenschleimhaut entnommen wurden. Material und Methoden: Diese Fallserie schloss drei Patienten ein, bei denen sechs standardisierte BGT entnommen wurden. Ein erfahrener Parodontologe entnahm die Transplantate mithilfe von zwei unterschiedlichen Methoden. Nach der Abgrenzung der festgelegten Transplantatgröße durch Inzisionen wurde das Transplantat der einen Seite noch vor der Entnahme mit einem kugelförmigen Diamantschleifkörper entepithelisiert. Das kontralaterale Transplantat wurde erst entnommen, und dann extraoral mit einem 15C-Skalpell entepithelisiert. Nach Abschluss der chirurgischen Maßnahmen wurde die histologische Untersuchung durchgeführt. Ergebnisse: Bezüglich der Dicke, den Proportionen und der Zusammensetzung zeigten sich nach Anwendung der beiden untersuchten Techniken keine offensichtlichen Unterschiede zwischen den Transplantaten. In fünf von sechs Fällen waren nach der Entepithelisierung noch Epithelreste erkennbar. Trotz größerer Techniksensitivität lieferte die Epithelentfernung mit dem rotierenden Instrument bessere Ergebnisse. Als Vorteile der konservativen Epithelexzision mit dem Skalpell sind eine bessere Handhabung und Ebenmäßigkeit des Transplantats zu nennen. Schlussfolgerungen: Trotz der breiten Anwendung und großen Vielfalt der üblicherweise verwendeten Techniken für die Entepithelisierung von Transplantaten, gehen die Autoren davon aus, dass mit Skalpellen in den seltensten Fällen eine vollständige Epithelexzision erreicht wird. Anders als die unzuverlässige Entepithelisierung können als große Vorteile der Skalpelltechnik die Handhabung und die Ebenmäßigkeit des Transplantats genannt werden. Dagegen scheint die vorgestellte neue In-situ-Entepithelisierung mit einem kugelförmigen Diamanten besser vorhersagbare Ergebnisse zu liefern.
